Bring current rabies vaccination certificate for each dog along with a Dog Park Registration form and Liability Waiver signed by all dog handlers to one of the the following locations. You may ask your vet to fax your dog's rabies certificate to 515-239-5355 or email amesparkrec@cityofames.org.

If the rabies certificate we have on file is not current, please attach a copy along with this form. This information was on the renewal letter we emailed in November.
Please attach your dog's current rabies vaccination certificate to the email of your Facility Use Permit or Waiver Form. If you don't have an electronic copy, please bring the certificate when you purchase your tag. We cannot assign a tag to your dog without this certificate on file. Thank you! |

PLEASE NOTE: We will begin winterizing the park system on October 15 of each year. This includes shutting off the water. If you visit a park after this date, the water fountains and restrooms will not be available for use; this includes the Dog Park. Please remember to bring water and a bowl for your dog when you visit the dog park. Thanks!
